The cultist shifts, not enough to avoid getting hit by the sword but at least it doesn't hit him in any vital parts. He lets a cry of pain out when it hits his own shoulder but at least he's done the hardest part. The knife is in, now he only has to make sure the kid is infected.

So he forces himself through the pain and twists the knife inside the kid's shoulder a bit.
