There is a second man, one who is only just now making his way inside with a bag full of groceries. He takes one look at the scene and moves around the detective to set the groceries on the table. At the moment, John doesn't particularly seem all that concerned with the fact that Sherlock is doubled over in pain or groaning.

"Honestly Sherlock, you should be the one apologizing to her for sneaking up on her." John is actually familiar with Ran - met her and had a few cooking lessons from her though she won't remember that.

So he turns to her and holds out a hand to greet her properly. "Sorry about him. John Watson. You're a friend of Conan's?"
